Student Health Insurance


Student Health Insurance

Chicago State University is pleased to offer an Injury and Sickness Insurance Plan from a major insurance provider. Full-time students (Undergraduate 12 hours / Graduate 9 hours / Pharmacy 12 hours / Doctorate 6 hours, or more, during each of the fall or spring semesters) are enrolled by the university in a State of Illinois-mandated health insurance plan. Students will be billed every semester for the cost of the plan. Students who can provide evidence of equal or better coverage than offered in the plan have the option of waiving this requirement by the cancellation deadline announced prior to the beginning of each semester. NO EXCEPTIONS can be made to published deadlines! All registered graduate students taking 9 or more credit hours, and doctoral students taking 6 or more credit hours, are required to participate in this plan on a hard waiver basis. The fee applies to all on-campus credit hours and to a combination of off-campus and on-campus credit hours. All international students are required to purchase this insurance plan for fall, spring, and summer terms unless they already have a qualifying health insurance plan as approved by the Wellness Health Center. Students will need to provide proof of their current qualified plan each semester they are enrolled in order to waive the mandatory health insurance.

Part-time students taking 6 to 11 credit hours may participate in this plan on a voluntary basis.

Students must actively attend classes for at least the first 31 days after the date for which coverage is purchased. Home study, correspondence, Internet, and television courses do not fulfill the eligibility requirements for mandatory enrollment. Note that students enrolled in all web/online courses are not charged for the insurance plan. The company maintains its right to investigate student status and attendance records to verify that the policy eligibility requirements have not been met. If the company discovers the eligibility requirements have not been met, its only obligation is a refund of the premium.

Insured students may also enroll their eligible dependents in the health plan. Eligible dependents are the student’s legal spouse or domestic partner and any dependent children under 26 years of age. To enroll dependents, please visit Cougar Connect for information on enrolling.

Students are eligible to waive the mandatory health insurance charge if they can provide proof that they are insured under comparable coverage. Students are encouraged to review the plan benefit brochure on the Health and Wellness Center site before completing a waiver form. Students should check their CSU email for the website address and instructions to complete a waiver/enrollment form 72 hours after being enrolled full-time. The Office of Student Health Insurance can be contacted if a student has not received the email within 72 hours after registering as full-time student.

Upon completion of the online process, the student will receive a confirmation of approval or denial via the student’s CSU email. A copy of this email confirmation should be retained for the student’s records. The confirmation email that the student will receive after submitting the online waiver will be required for any waiver dispute. The student must complete a waiver/enrollment form for each fall and spring semester that the student is enrolled full-time. The waiver/enrollment form is valid for one semester only. Failure to submit an online waiver form will obligate the student to pay the coverage listed on the student’s tuition bill. Waiver/Enrollment deadlines are published in the General Information document each semester.

NOTE: Students should not wait until they receive a bill with the insurance charge itemized, or they may miss the deadline to cancel. Cancellation requests are not accepted after the cancellation deadline, which is published in the General Information document each semester.

 

Waiver/Enrollment Process

If you have insurance: Students who are adequately insured have the option of waiving out of the mandatory health plan and having the charge removed from their tuition bills. Failure to submit an online waiver or enrollment form by the deadline will obligate students to pay the Full-time Student Insurance Premium coverage indicated on your tuition bill.  If your waiver is denied, and you would like to be reconsidered, you may provide proof of coverage to the Office of Student Health Insurance.

If you are not insured: Full-time students who do not have insurance must purchase the mandatory insurance provided through the university.

Each Fall and/or Spring semester, students are required to complete an online waiver form to opt out or complete an online enrollment form before the online waiver/enrollment deadline. Failure to submit an online waiver form by the deadline will obligate students to pay the SHI premium fee itemized on the tuition bill. A confirmation e-mail is sent after the waiver is submitted. The confirmation e-mail will be required for any waiver disputes.

Enrollment Process

Full-time students who wish to enroll in the SHI plan should complete an online form before the waiver/enrollment deadline. Part-time students who wish to enroll, read the information below for “Part-time students, spouses, and dependents eligible for coverage.”

Students Who Take No Action: E-mails are sent to every student who is charged for the university’s mandatory SHI plan. Students are required to submit an online waiver or enrollment form. Students who do not submit an online waiver/enrollment form by the specified waiver enrollment deadline will be required to purchase the university’s mandatory SHI plan. After the waiver/enrollment period has expired, the SHI premium fee cannot be refunded.

Part-Time Students, Spouses, and Dependents Eligible for Coverage

Part-time students who enroll in 6-11 credit hours of coursework are allowed to voluntarily enroll with United Health Care into the student health insurance plan provided through the university. Eligible students who do enroll may also insure their Dependents. Eligible Dependents are the student’s legal spouse, partner by civil union, Domestic Partner, and dependent children under 26 years of age. Dependent eligibility expires concurrently with the Insured student.
